Sustainability and Transformation Plans (STP) progress tracker

April 2017 will see a huge change to the way NHS services are commissioned. STPs are pivotal to this change that critically impacts your business.

At a time of austerity for many public services, additional funding is available to the NHS. However a condition of this funding is using a different approach to care which will be directed by Sustainability and Transformation Plans (STPs).

How will STPs change NHS service provision?

STPs will see the commissioner/provider split replaced by local planning and management within 44 areas called ‘footprints’. These area-specific, whole-system plans will identify the health needs of their populations, and how to address them quickly and effectively at a local level.

Tracker For Me Proof V1

What does the STP tracker contain?

The STP tracker contains an interactive STP map, with links to each of the 44 local NHS ‘footprints’. Each ‘footprint’ contains the following information:

  • Priorities and objectives, both top line and therapeutic
  • Key personnel and committees
  • Relevant organisations
  • Budgetary information
  • Population statistics
  • Links to documents and websites for further information

In addition, it will report on the 10 questions the STP must answer to maintain their funding streams:

  1. How are you going to prevent ill health and moderate demand for healthcare?
  2. How are you engaging patients, communities and NHS staff?
  3. How will you support, invest in and improve general practice?
  4. How will you implement new care models that address local challenges?
  5. How will you achieve and maintain performance against core standards?
  6. How will you achieve our 2020 ambitions on key clinical priorities?
  7. How will you improve quality and safety?
  8. How will you deploy technology to accelerate change?
  9. How will you develop the workforce you need to deliver?
  10. How will you achieve and maintain financial balance?
